Most Advantage HMO’s and PPO’s have a network of doctors, hospital,
and clinics. Patients are allowed to use other doctors and facilities
in an emergency. Co-pays and deductibles may be charged for office visits,
tests, x-rays, surgeries, hospital. There is no way to determine the
charges before treatment. Most plans will have a maximum out of pocket
amount per year possibly $3,000.00. There may not be any limit to the
out of pocket charges if the patient goes outside of the network. If
a subscriber remains on the plan longer than a year, they may not qualify
